Support & Services
There are a number of hospice and palliative care services and supports across eastern Ontario, many of which can be accessed through the Champlain local health integration network (LHIN), day hospice programs, visiting volunteers, residential hospice, family physicians, palliative care units, teams and specialists.
Day Hospice Programs
The Palliative Day Program is a day away in a home-like setting, where clients can participate in activities such as arts & crafts, massage and reiki therapy, music, etc. as well as social and emotional support from volunteers, staff and other clients while providing some respite to their families.
Family Doctors
Whether working in solo practice, in a Family Health Team, a Community Health Centre or other group practice, family doctors and other primary care providers form the basis of quality palliative care.
There are specialized palliative care services and supports available in the region including some physician groups who focus on providing palliative care (including the Community Palliative Medicine Associates, Central Palliative Group, Rideau Palliative Group and Soins Palliatifs Orleans) as well as the Regional Palliative Consultation team. These services require a referral from a physician or healthcare provider.
In-Home Visiting Volunteers
Volunteers provide emotional support and companionship for clients while offering respite to family members in a setting of their choice. Volunteers can share in activities such as going on walks and outings, reading, and shared hobbies and interests.
Residential Hospice
When being at home is no longer possible, consider residential care in a home-like and peaceful setting, where an interdisciplinary team provides physical, emotional and spiritual support with a focus on pain and symptom management and quality of life.
