Community Palliative Medicine Groups support patients in Central, Western and Eastern Ottawa, as well as surrounding regions such as North Grenville, Kemptville, Merrickville, Prescott, Russell and Cornwall.

Referrals must be completed by a physician.

Soins Palliatifs Orléans (SPO) / Orléans Palliative Care Team provides bilingual community palliative care to patients.

Catchment Area: Orléans and Cumberland Ontario, including area codes: K1C, K1W, K1E, K4A, K4B, K4C, K0A, depending on proximity to Orleans.

  • early integration of palliative care, in conjunction with disease modifying treatments
  • accepting referrals for patients with life-limiting illnesses who would benefit from being supported by 24 hour/day palliative care services in the community
  • provides palliative care consultation services in the community and can take the role of most responsible provider

The Community Palliative Medicine Associates (CPMA) is a partnership of family physicians dedicated to delivering quality in-home palliative care to patients with a life-limiting illness.

Their mission is to provide holistic patient centred medical care and symptom management support in an effort to relieve suffering and enhance the quality of life for patients and their families.

A group of physicians based in North Grenville that provide palliative symptom management and end-of-life care.

Catchment Area:  North Grenville, Kemptville, Merrickville, Winchester, Winchester Springs, Chesterville, Spencerville, North Gower, Richmond, Osgoode, Kars, Manotick, Greely, Metcalfe. Addresses just outside these boundaries may be considered.

United Counties of Prescott Russell (UCPR) Palliative Outreach Service

The United Counties of Prescott Russell Palliative Outreach Service is dedicated to providing dignifying palliative symptom management and end-of-life care.

Services are provided in both English and French.

Catchment Area:

  • Communities along highways 417 & 17 (Vankleek Hill, Hawkesbury, L'Orignal) to the Quebec border
  • Communities along highway 401 (Bainsville, South Lancaster, Summerstown, Cornwall) from the Quebec border to Cornwall
