Symptom Response Kit
Symptom Response Kits are available for palliative care clients who:
-require access to medications and supplies
-are in urgent palliative care situations in the home
What is included:Â
The Symptom Response kit includes enough medications and related medical supplies to manage the palliative care client’s distressing symptoms in the home for a 24 to 72 hour period.

Palliative injectable medications for Symptom Relief Kit (SRK) – Renewal, Refill or Medication Addition Form
Use this form when a patient already has a Symptom Relief Kit (SRK) in the home and requires a medication renewal, refill, or addition. The standard OHaH SRK order form should not be reused for these requests.
This form helps streamline ordering and ensures that required information—including appropriate medication concentrations, quantities and LU codes—is included.
To use: Complete and sign the form, then fax it to Ontario Health atHome (OHaH) for processing.
