In Ottawa, all referrals for inpatient hospice palliative care are triaged by the Central Referral and Triage (CRT). Patients are triaged to one of the following teams based on their level of care:
- Palliative Care Unit at Bruyère Continuing Care
- Hospice Care Ottawa
- Palliative Care Home Support with Ontario Health atHome
Make a referral and admit a patient in Ottawa
To refer an inpatient to a hospice palliative care bed, a physician must:
- Review Hospice Palliative Care admission criteria
- Encrypt and send a completed Hospice Palliative Care Admission form to BruyereClinicalAdmissions@bruyere.org or fax to 613-562-6095.
The referral will be triaged by the CRT team— a partnership between Bruyère Continuing Care, Ontario Health atHome, and Hospice Care Ottawa.

Bruyère's Palliative Care Unit
The referring physician can contact Bruyère telecommunications at 613-562-6262 and request that the on-call staff MD for Palliative Care be paged. This is available any day of the week after 16:00 and on weekends.
The referring physician will discuss the details of the case with the on-call Palliative Care MD to determine if an urgent admission is warranted.
Hospice Care Ottawa
The referring physician can contact the on-call nurse at 613-260-2906 ext. 3. This is available any day of the week after 16:00 and on weekends.
