Community Palliative Medicine Groups support patients in Central, Western and Eastern Ottawa, as well as surrounding regions such as North Grenville, Kemptville, Merrickville, and Cornwall.
Referrals must be completed by a physician.
Referrals can also be made directly with the specific group using the information below.

- early integration of palliative care, in conjunction with disease modifying treatments
- accepting referrals for patients with life-limiting illnesses who would benefit from being supported by 24 hour/day palliative care services in the community
- provides palliative care consultation services in the community and can take the role of most responsible provider

Community Palliative Medicine Associates
The Community Palliative Medicine Associates (CPMA) is a partnership of family physicians dedicated to delivering quality in-home palliative care to patients with a life-limiting illness.
Their mission is to provide holistic patient centred medical care and symptom management support in an effort to relieve suffering and enhance the quality of life for patients and their families.

The Ottawa South Palliative Group's (OSPG) objective is to provide patient centred in-home care to patients with life limiting conditions. Their team consists of Palliative Care Specialists and Family Physicians with a special interest in Oncology and Palliative Care. Â
Their mission is to help manage symptoms of end of life (such as pain, shortness of breath, and nausea) with a focus on comfort care and quality of life.Â

A group of 5 physicians providing palliative care services in the communities covering much of Cornwall and the surrounding area (Part of South Dundas, Part of North Glengarry, and Stormont counties - call for specific locations which may not be covered).
Services are provided in both English and French.

- Communities south of highway 417 (Crysler, Maxville, Alexandria, to the Quebec border).
